All in all it looks great, Rachel. I want mention one quick thing right away, though: I think the 'clear' button form control is usually useless and often harmful. The aggravation of clearing a form you've painstakingly filled out far outweighs the convenience of being able to clear a form. I would recommend leaving it out of any future Koha template.
