Some enhancements to the patron account pages.

Convert barcode to itemnumber in maninvoice, mancredit.
flagsrequired changed to updatecharges.
change desciption in maninvoice if the invoice type is new card.
couple more output fields in boraccount.
expand writeoff() in pay with stuff for accountoffsets table.
