After applying the patch, the "create an item record when receiving this
serial," "do not create an item recodr when receiving this serial", "Staff and
Public Display", "Number of Issues to display to Staff," and "Number of issues
to display to the public" labels are all still too narrow. I did ctrl-f5 to
force a complete refresh.

The problem where the numbering patterns were not appearing is fixed by the
patch.
