Problem reported in IRC chat on Nov. 5, 2009.

Koha 3.0.1 user reported duplicate biblio records being created each time
cataloging was done (creating new from Z39.50 search) in Internet Explorer. 
The duplicate records had adjacent biblionumbers, but whether the itemless
record came first or second could not be reliably determined.

Investigation showed that using Firefox did not produce this error.  Initially,
Internet Explorer threw up a "duplicate warning sign", then after the computer
was upgraded, the sign went away and records were automatically duplicated.

Open question:  is this error reproducible in later versions of Koha?
