Bug 26646: Group holds to pull by item rather than bib

To test:
1 - find a bib with 2 items
2 - place an item-level hold on item 1 for patron A
3 - place an item-level hold on item 2 for patron B
4 - go to Holds to Pull
5 - it shows one entry for the bib, indicates 2 items should be pulled, but
only lists patron A
6 - apply patch, restart services
7 - reload Holds to Pull, confirm it now shows two entries, one for each item
