Adding a system preference to customize the search form was just an idea to make it easier for system administrators to customize some of the layout, however, it might also be adding an unnecessary level of complexity to the code. I agree with you Nicole, I think it is fine to setup a search form that lets you search the fields associated with patron records without having a system preference.

This is certainly something I have heard several people ask for.  I
personally like your option #1.  I'm not sure why we'd need a system
preference for this though.  Why wouldn't we just have an advanced
search form that lets you search the fields associated with patron
records?
