Hi,
I thought the idea of moving OPACItemHolds to the circulation matrix was
promising in terms of more flexibility:

And there is another patch implementing item level holds a bit different
especially for serial records:
http://bugs.koha-community.org/bugzilla3/show_bug.cgi?id=11157
I think there would definitely be interest in such a feature.
