An additional example, it's not terribly uncommon for people to go by
their middle name, rather than their first (I can think of two close
relations who do this) and so this lets them be called by what they
prefer.

It seems that you're using in a fairly different way (specifically, with
surnames.) I think making it a syspref would not be difficult at all.
