Hi Sean,

the SQL files in the installer directory will only be run when first
installing Koha, but not again when updating. If you want to add
additional languages later, you have to alter the database directly at
the moment.

There is currently an open bug report for adding additional languages:
bugs.koha-community.org/bugzilla3/show_bug.cgi?id=15067

Maybe looking at the patch there will be useful too.
